Opioid Settlement Funds
As part of the statewide settlement in the Purdue Pharma opioid lawsuit, the City of Fairmont was awarded a total of $626,159 in funds to date. These are to be used to address or minimize the impact of opioid addiction in our community.
The City's grants administrator, in collaboration with the City Attorney, has created an application process that will allow entities to apply for grants from the total sum of money awarded to the City.
Applications will be accepted for up to $100,000.00 for direct services and $50,000.00 for indirect service projects. Funds are limited due to the amount of settlement received, and projects may not receive the full request.
A 20% match of funds will be required for all projects; however, a waiver of match may be requested and will be evaluated on a case-by-case basis. Waiver Request forms are available upon request. Funding decisions are at the sole discretion of the City, and the City reserves the right to deny any request for funds and/or waivers of match for any reason.

The Application Process
The application process consists of three steps. These steps must be completed in order to ensure that an application is properly submitted.
Step One
The first step requires that applicants review the West Virginia First Foundation's Memorandum of Understanding. This MOU lays the groundwork for the rest of the application and contains information that is needed to complete sections later in the process. The MOU can be found in the City's digital Document Center for review and download.

Step Two
The second step is the City of Fairmont Opioid Grant Application. This form can be completed digitally, or it can be printed and physically completed. This form requires information that is provided in the MOU linked above. Please read the form's instructions carefully.

Step Three
The third step is a suite of compliance agreements required for the City to issue grant funds to outside organizations. This packet requires several signatures and other information, so please take time to review the requirements outlined by each form in the packet. Several of the forms also require a notary's approval. The City has several notaries on staff that can assist with this verification step if necessary.

Submission
Once all the steps above have been completed, applications can be submitted digitally via email to the City's grant administrator, Rene Graves, at rgraves@fairmontwv.gov. Applications can also be submitted physically to the City of Fairmont at our offices located at 200 Jackson Street, Fairmont, WV 26554. We're on the third floor of the J. Harper Meredith Building.
The Review Process
Once the applications have been submitted, the City's Ad Hoc Opioid Fund Review Committee will convene to review applications and score them based on the rubric below.
